If you are faxing or mailing the form, please return with payment information to MPA - The Association of Magazine Media. Keep a copy for your records. Payment must accompany your form before it will be processed.  Checks must be accompanied by a copy of the form.

Completed forms with payment and all guest names must be received by Thursday, April 19.

Table Assignments:
Table assignments will be emailed to the contact listed on the form one week prior to the event. Purchasers of individual tickets will receive table assignments upon check-in at the event. Physical tickets will not be distributed for this event.

Refund/Cancellation Policy:
Ticket fees are non-transferable.  Excluding a processing fee of $100, full refunds will be granted if written requests are received by MPA/ASME prior to April 24, 2012.  No refunds will be given thereafter.  Verbal cancellations will not be accepted.  If a form is received (by fax or mail) without payment, and subsequently cancelled, you will be invoiced for the appropriate amount.   Substitutions will be accepted at any time with written notification.  No refunds for no-shows.  MPA/ASME reserves the right to change without notice any statement in this publication concerning, but not limited to, rules, policies, fees, curricula and speakers.

A 3% processing fee will be added to tickets purchased with a credit card.

Amounts paid to MPA/ASME may be deductible as a business expense, but are not deductible as a charitable contribution.  FEIN: 13-1087160.
